Life-changing and life-affirming, at Outreach’s Recovery Residence, clients from Nassau and Suffolk Counties can heal, grow, and develop in all life roles – get well, stay well, build a healthy life. Specialized services for women in our Brentwood program offers: enhanced gender responsive outpatient treatment for women utilizing a trauma-informed framework, including psychiatric evaluation and mental health monitoring, specialized group sessions (anger management, life skills, relapse prevention), domestic violence counseling, recovery planning, family counseling, health education, vocational assessment and ongoing support services. Participation in an outpatient program is required, and transportation is available for residents to and from Outreach’s outpatient program in Brentwood, which provides enhanced outpatient treatment for women. Length of stay varies by each resident’s needs and progression in outpatient treatment. This brand new residence offers two-person rooms with single beds furnished with desk spaces for privacy and work, designated community spaces for family meetings and sober social gatherings, brand new kitchen and communal dining area, and an outdoor patio and garden area. Opening its doors in January 2020, with official ribbon-cutting several months later, the 25-bed Recovery Residence for Women is a 24-hour staffed, safe and supportive living facility for women and women with children who have completed intensive residential or inpatient treatment and are transitioning back to community living. Outreach’s Recovery Residence for Women is centrally located on Outreach’s Crooked Hill Campus in Brentwood, Suffolk County. Discovery Mood & Anxiety Program at Vista is a six-bed residential treatment program for female identifying and non-binary (regardless of sex assigned at birth) adolescents ages 11-17 who are struggling with their mental health. See attached LIC 809-D pages.Īn exit interview was conducted, appeal rights provided, and a copy of this report left with Mental Health Technician Latiesha O'Neal, whose signature on this form confirms receipt. LPA is issuing three (3) citations of California Code of Regulations, Title 22, Division 6, Chapter 1, Article 6, Section 80061 Reporting Requirements, Section 80044 Inspection Authority of the Licensing Agency, and Chapter 5, Article 6, Section 84065 Personnel Requirements. The facility was previously issued an advisory on that the facility needs to ensure there is always a facility manager present when children are present whenever the administrator is absent and that the facility may designate more than one facility manager at a facility. The therapist was in a session and not able to provide supervision on the floor nor represent the facility during today's visit. Lastly, during today's visit, LPA was informed that Interim Administrator Virginia Parish is out on sick leave, and the facility manager is working "virtually." There are currently four (4) residents and three (3) staff, including two (2) direct care staff (mental health technicians) and one (1) therapist. The investigator went over the Licensing agency's inspection authority and was still denied. Additionally, during the investigation, an investigator with the Licensing agency's Investigations Branch (IB), after introducing self and requesting to interview residents, was denied permission to do so. LPA met with Mental Health Technician Latiesha O'Neal and informed of the purpose of the visit.ĭuring an investigation at the facility, it was discovered that a youth made a disclosure of inappropriate sexual behaviors from a staff toward the youth, but the facility did not report this incident due to not believing the youth's statement. On 7/1/22, Licensing Program Analyst (LPA) Mariah Hawkins made a visit to the residential treatment group home facility for a case management visit.
